Ellett memorial Hospital serves as the custodians of all patient health information. The Health Information Management (HIM) Department develops policies, procedures, systems, and safeguards to ensure that patient health information is documented, maintained, and disclosed in accordance with all state and federal laws, regulations and standards. We constantly strive to uphold the accuracy, integrity, reliability, completeness, and timeliness of documentation which will help ensure the safety and quality of care provided to our patients. Through these efforts, the HIM department aids your physician and caregivers in providing excellent patient care.
Ellett Memorial Hospital complies with the Health Insurance Portability and Accountability Act (HIPAA) regulations concerning the release of protected health information (PHI). Upon request by the patient for medical information (medical records), the Health Information Management (HIM) Department will provide the patient’s medical records if a signed release form has been obtained with proper identification verified.
Authorization forms may be delivered in person to the HIM Department. Records will not be released until identification has been verified.
The HIM office is open Monday through Friday 8a.m. to 4:30p.m. For questions regarding your medical records, please call during regular business hours.
Ellett Memorial Hospital may charge a fee to copy the requested health information. The cost is per page with a handling charge with additional notary fees if applicable.
Request for health information will be responded to within 30 days of the receipt of your request.
